Overview

Glucagon-like peptide-2 (GLP-2) is a 33-amino-acid intestinotrophic hormone secreted by intestinal L cells. Its analogs form a genuine, clinically established drug class: teduglutide, a recombinant GLP-2 analog engineered for a longer half-life, is approved in several regions for short bowel syndrome. Additional long-acting investigational analogs, including apraglutide and glepaglutide, have reached late-stage clinical trials.

Mechanism

GLP-2 receptor activation promotes intestinal crypt-cell proliferation and villus growth, reduces epithelial apoptosis, and increases the absorptive surface and barrier function of the gut. These effects translate into improved nutrient and fluid absorption in patients with short bowel syndrome.

Dosing

Approved teduglutide is given as a daily subcutaneous injection with the regimen specified in its prescribing information. Investigational long-acting analogs are designed for less frequent administration; dosing in trials follows the published study protocols.
